Metastatic brain tumors are located in regions of the brain densely packed with critical neural functions — speech, motor control, memory — making the boundary for surgical resection extremely difficult to judge: too large a resection risks paralysis, speech loss, or disability, while too small a resection risks leaving residual cancer cells that lead to recurrence. The photodynamic therapy being advanced by Japan's National Cancer Center offers a new approach to this dilemma:
Before surgery, the patient is injected with the photosensitizer Lasallphine, which has a strong tendency to selectively accumulate in tumor tissue. After the surgeon removes the main tumor mass, a weak laser of a specific wavelength is used to illuminate the entire surgical cavity; the photosensitizer, activated by the laser, generates reactive oxygen species within any remaining cancer cells, precisely eliminating microscopic residual lesions while causing minimal damage to healthy brain tissue and important nerves that haven't absorbed the drug. This approach helps reduce the rate of recurrence after brain metastasis surgery while preserving the patient's quality of life as much as possible.
The Significance of "Advanced Medical Care B" Designation
Lasallphine-based photodynamic therapy has already received formal Japanese national health insurance coverage for primary malignant brain tumors (such as glioma), and the underlying technology is relatively mature. This new clinical trial for "metastatic brain tumors" has been classified by Japanese authorities under the "Advanced Medical Care B" framework, meaning the therapy's safety and cutting-edge efficacy have already passed rigorous review by the Ministry of Health, Labour and Welfare.
